El dolor pot ser produ\u00eft per un trauma, per\u00f2 moltes vegades apareix sense cap lesi\u00f3, pot ser agut o cr\u00f2nic. \u00c9s molt habitual en la pr\u00e0ctica cl\u00ednica di\u00e0ria tenir pacients amb dolor, afectant a totes les edats per\u00f2 sobretot a la poblaci\u00f3 femenina i a partir dels 60 anys.<\/p><p>La Medicina Occidental convencional davant d\u2019un dolor i una inflamaci\u00f3 actua habitualment amb medicaments analg\u00e8sics per inhibir les prostaglandines responsables de que es manifesti i de que es propagui.<\/p><p>Des de temps molt remots ja consideraven l\u2019acupuntura com un analg\u00e8sic. Els meridians que recorren el cos coincideixen amb estructures nervioses i vasculars. La energia (Qi), i la sang circulen per tota aquesta xarxa de canals i quan s\u2019estimulen punts d\u2019acupuntura, s\u2019origina un mecanisme neurofisiol\u00f2gic d\u2019inhibici\u00f3 del dolor.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-c2655b3 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"c2655b3\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-21b7fcb\" data-id=\"21b7fcb\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-032fb10 elementor-widget elementor-widget-text-editor\" data-id=\"032fb10\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong><u>Mecanisme del dolor<\/u><\/strong><\/p><p>El dolor comen\u00e7a quan els receptors localitzats al sistema extern de la pell s\u00f3n excitats, produint unes substancies anomenades prostaglandines,\u00a0 i posteriorment\u00a0 es condueix per les fibres nervioses m\u00e9s internes, manifestant un dolor agut i localitzat, o m\u00e9s lleu i dif\u00edcil de localitzar, que moltes vegades tamb\u00e9 va relacionat amb la inflamaci\u00f3.<\/p><p><strong><u>Teoria de la Porta de Control<\/u><\/strong><\/p><p>Melzack i Wall (1965),\u00a0 han aportat una explicaci\u00f3 cient\u00edfica per entendre el mecanisme d\u2019acci\u00f3 de l\u2019acupuntura en el dolor, la teoria de la <strong><em>Porta de Control<\/em><\/strong>.\u00a0 El trajecte del dolor que s\u2019inicia al Sistema Nervi\u00f3s Parasimp\u00e0tic (SNP) com pot ser un dit o un genoll, fa una aturada en una estaci\u00f3 a nivell de la medul\u00b7la espinal i continua en direcci\u00f3 al cervell.\u00a0 Al aplicar l\u2019acupuntura es crea un obstacle en aquesta estaci\u00f3, de tal manera que impedeix el cam\u00ed patol\u00f2gic del dolor bloquejant-lo.\u00a0 L\u2019acupuntura tamb\u00e9 fa segregar el nivell d\u2019endorfines del cos, augmentant aix\u00ed el llindar del dolor.<\/p><p><strong><u>Segons el tipus de teixit afectat<\/u><\/strong><\/p><ul><li><strong>Muscular:<\/strong>\u00a0 Contractura muscular, Contusi\u00f3 muscular. Els dolors musculars empitjoren amb el moviment.<\/li><li><strong>Tendons:<\/strong>\u00a0 Tendinitis<\/li><li><strong>Lligaments:<\/strong>\u00a0 Esquin\u00e7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0<\/li><li><strong>Articulaci\u00f3: <\/strong>\u00a0Artritis, Artrosis, Bursitis<\/li><li><strong>Nervi\u00f3s:<\/strong>\u00a0 Neur\u00e0lgia<\/li><\/ul><p><strong><u>Definici\u00f3 del tipus de disharmonia<\/u><\/strong><\/p><ul><li><strong>Contractura muscular:<\/strong>\u00a0 Contracci\u00f3 persistent i involunt\u00e0ria del muscle.<\/li><li><strong>Contusi\u00f3 muscular:<\/strong>\u00a0 Trauma directe amb possible ruptura de vasos sanguinis podent provocar hematomes.<\/li><li><strong>Tendinitis:\u00a0 <\/strong>Es la inflamaci\u00f3 i\/o posterior degeneraci\u00f3 del tend\u00f3, teixit no contr\u00e0ctil que uneix l\u2019os i el m\u00fascul.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0<\/li><li><strong>Esquin\u00e7:<\/strong>\u00a0 Distensi\u00f3 o estirament excessiu d\u2019un lligament, teixit el\u00e0stic que uneix les dos parts \u00f2ssies d\u2019una articulaci\u00f3.\u00a0 Es produeix despr\u00e9s d\u2019un cop brusc, caiguda o torcedura.\u00a0<\/li><li><strong>Artritis:\u00a0 <\/strong>\u00c9s la inflamaci\u00f3 d\u2019una articulaci\u00f3.\u00a0 Existeixen diferents tipus d\u2019artritis per\u00f2 una d\u2019elles \u00e9s la Artritis Reumatoide,\u00a0 d\u2019origen autoimmune.<\/li><li><strong>Artrosis:<\/strong>\u00a0 \u00c9s la degeneraci\u00f3 del cart\u00edlag d\u2019una articulaci\u00f3.\u00a0 \u00c9s habitual en les articulacions de la columna vertebral, genolls, malucs i espatlles.\u00a0 Sol passar en gent a partir dels 60 anys, i afecta tant a homes com a dones.<\/li><li><strong>Bursitis:<\/strong>\u00a0 Inflamaci\u00f3 de la bursa, bossa que cont\u00e9 l\u00edquid i que forma part de l\u2019articulaci\u00f3.<\/li><li><strong>Neur\u00e0lgia:\u00a0 <\/strong>Dolor o sensaci\u00f3 en el recorregut d\u2019un nervi. La neur\u00e0lgia del trigemin i la ci\u00e0tica s\u00f3n exemples de les habituals.<\/li><\/ul><p><strong><u>Segons la localitzaci\u00f3 del dolor<\/u><\/strong><\/p><ul><li><strong>Columna vertebral:<\/strong>\u00a0 cervic\u00e0lgia, lumb\u00e0lgia, dors\u00e0lgia, ci\u00e0tica.<\/li><li><strong>Espatlla:<\/strong> Espatlla congelada, tendinitis, bursitis, artritis, artrosis, inestabilitat articular.<\/li><li><strong>Colze:<\/strong>\u00a0 colze de tenista, colze de golfista.<\/li><li><strong>Monyeca:<\/strong>\u00a0 S\u00edndrome del t\u00fanel carpi\u00e0.\u00a0<\/li><li><strong>M\u00e0:<\/strong>\u00a0 artrosis, artritis, tendinitis.\u00a0<\/li><li><strong>Maluc:<\/strong> artrosis, bursitis, ci\u00e0tica.<\/li><li><strong>Genoll:<\/strong>\u00a0 artrosis, artritis,\u00a0 patologia\u00a0 dels lligaments, esquin\u00e7 del menisc, patologia de la r\u00f2tula.<\/li><li><strong>Turmell:<\/strong>\u00a0 esquin\u00e7 del turmell, tend\u00f3 d\u2019Aquil\u00b7les.<\/li><li><strong>Peu<\/strong>:\u00a0 fascitis plantar, gota.<\/li><\/ul><p><strong>S\u00cdNDROMES DES DE LA MEDICINA TRADICIONA XINA<\/strong><\/p><p>Des de la perspectiva de la Medicina Tradicional Xina, el dolor que afecta al sistema muscular, als tendons i a les articulacions, s\u2019anomena <strong><em>s\u00edndrome Bi<\/em><\/strong> o <strong><em>s\u00edndrome d\u2019obstrucci\u00f3 dolorosa<\/em><\/strong>.\u00a0 El <em>s\u00edndrome Bi<\/em> pot presentar dolor, entumiment, pesades i dificulta la mobilitat en els meridians localitzats al llarg de l\u2019organisme. Fer una valoraci\u00f3 \u00e9s primordial per poder diferenciar quins s\u00f3n els canals afectats, poder trobar el motiu de la disharmonia, i poder-ho tractar d\u2019una manera m\u00e9s eficient.<\/p><p>Els diferents s\u00edndromes de disharmonia que podem trobar en el <strong><em>S\u00edndrome Bi<\/em><\/strong>, s\u00f3n els seg\u00fcents:<\/p><ul><li><strong>Estancament de Sang<\/strong>:\u00a0 Dolor fort, punxant i localitzat, que pot arribar a immobilitzar la zona afectada. Les mol\u00e8stia augmenten durant la nit.\u00a0 Pot venir precedit d\u2019un estancament de Qi.<\/li><li value=\"2\"><strong>Estancament de Qi<\/strong>:\u00a0 Dolor amb espasmes i contraccions.\u00a0 Relacionat molt directament amb l\u2019estr\u00e8s.<\/li><li value=\"3\"><strong>Atac extern de factors pat\u00f2gens (vent, fred, calor i humitat)<\/strong>:\u00a0 Dolor que empitjora segons la climatologia externa.<ul><li><strong>Vent:<\/strong> dolor que es mou i va canviant de lloc.<\/li><li><strong>Fred<\/strong>: dolor intens i agut que millora amb l\u2019aplicaci\u00f3 de calor. Temor al fred.<\/li><li><strong>Humitat:<\/strong> dolor fix que empitjora en els dies plujosos o ennuvolats. Pot estar inflamat i manifestar edemes.<\/li><li><strong>Calor:<\/strong> Dolor amb inflamaci\u00f3, rubor i edema, que no tolera la palpaci\u00f3.<\/li><\/ul><\/li><li value=\"4\"><strong>Defici\u00e8ncia de Qi i sang<\/strong>:\u00a0 Segons la teoria dels cinc elements, el sistema muscular pertany a l\u2019element terra i la Melsa \u00e9s l\u2019encarregada de nodrir tota la musculatura del cos. Si la persona presenta una defici\u00e8ncia de Melsa o una defici\u00e8ncia d\u2019energia (QI) generalitzada, pot mostrar debilitat muscular per manca de nutrici\u00f3.Quan una musculatura est\u00e0 d\u00e8bil \u00e9s m\u00e9s f\u00e0cil que els factors externs com el vent, el fred i la humitat penetrin als teixits i produeixin dolor.<\/li><\/ul><p><strong><u>T\u00e8cniques per millorar el S\u00edndrome Bi des de la Medicina Tradicional Xina<\/u><\/strong><\/p><p>Despr\u00e9s d\u2019haver realitzat una valoraci\u00f3 en cada un dels casos, el terapeuta decideix quines s\u00f3n les t\u00e8cniques que utilitzar\u00e0 per poder tractar i eliminar el dolor.\u00a0 Habitualment se n\u2019utilitzen m\u00e9s d\u2019una alhora, alguna d\u2019elles com la acupuntura, la moxibusti\u00f3 i les ventoses s\u2019apliquen a la consulta, i d\u2019altres com la fitoter\u00e0pia, \u00e9s el pacient que s\u2019ho va prenent di\u00e0riament a trav\u00e9s de capsules, pastilles o decoccions.\u00a0<\/p><ul><li>Acupuntura<\/li><li>Moxibusti\u00f3<\/li><li>Ventoses<\/li><li>Massatge Tuina<\/li><li>Fitoter\u00e0pia Xinesa.<\/li><li>Electroacupuntura<\/li><\/ul><p>Segons la teoria dels cinc elements, a cada un dels \u00f2rgans li correspon un teixit, aix\u00ed si vull millorar la musculatura, els tendons, els lligaments o els ossos, actuar\u00e9 en el seu \u00f2rgan corresponent.<\/p><ul><li>Musculatura:\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Melsa<\/li><li>Tendons i lligaments:\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Fetge<\/li><li>Ossos:\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Rony\u00f3<\/li><\/ul><p>La fitoter\u00e0pia xina tamb\u00e9 s\u2019utilitza a nivell t\u00f2pic a trav\u00e9s de cataplasmes, ung\u00fcents, olis i cremes per tractar la zona danyada d\u2019una manera m\u00e9s directa.\u00a0 Tot aquest conjunt de remeis s\u2019han utilitzat durant molts anys a l\u2019antiga Xina, sobretot pels guerrers d\u2019arts marcials, en que durant els seus combats, eren habituals les contusions i ferides d\u2019arma blanca, i necessitaven una soluci\u00f3 r\u00e0pida i efica\u00e7 al moment.<\/p><p><strong>APLICACI\u00d3 DE GEL<\/strong><\/p><p>Tot i que la medicina convencional occidental aplica gel en molts dels casos quan hi ha hagut una contusi\u00f3 o inflamaci\u00f3, la Medicina Tradicional Xina no l\u2019utilitzar\u00e0 mai.\u00a0 El gel fa disminuir el dolor i baixar la inflamaci\u00f3 en el moment d\u2019aplicar-lo, per\u00f2 tamb\u00e9 bloqueja la zona, aix\u00ed que posteriorment el dolor tornar\u00e0 a sorgir, empitjorar\u00e0 i far\u00e0 m\u00e9s llarga la recuperaci\u00f3 o m\u00e9s cr\u00f2nica la situaci\u00f3.<\/p><p>Davant d\u2019una contusi\u00f3 si existeix dolor i inflamaci\u00f3 actuaren amb plantes i herbes xines fresques per poder desinflamar i baixar el dolor, i posteriorment quan ja no existeixi inflamaci\u00f3 ni estigui vermell canviarem el tractament per\u00f2 seguirem actuant per no cronificar la lesi\u00f3, ni que es converteixi en una zona sensible als canvis clim\u00e0tics com el fred o la humitat.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"<p>Musculoesquel\u00e8tic Els dolors musculoesquel\u00e8tics comprenen una gran varietat de trastorns inflamatoris i degeneratius de l\u2019aparell locomotor.
